• How do I schedule an appointment?

    There are two ways to schedule an appointment. You can go to the Patient Portal on our website or you can call our office 208-888-6886.
    How should I prepare for my initial visit?

    Please complete the new patient intake forms, questionnaires listed on the patient portal. (see link on website). Based on the reason for your visit, you may be asked to complete other forms to help prepare for the visit. We request that you complete the paperwork at least 5 days prior to your appointment. You may email forms to info@fmidaho.com or fax to our office 208-938-0355.

  • What can I expect from my initial visit?

    The office visit may take 60-90 minutes depending on the complexity of your health concerns and condition. She will take a comprehensive health history and review any previous lab testing or records pertinent to your health concerns. She may recommend specialty testing, prescribe medications, nutritional supplements, specific diets or referrals based on your medical history and needs.

    Follow up visits: Cynthia or Rebekah will review the initial results of any lab or specialized testing you have had completed. Depending on what your health care issues are she may prescribe medications, hormones, supplements, lifestyle interventions and or nutritional plans to address your specific concerns. The number of visits you will need greatly depends on your individual needs. The ultimate goal is for you to achieve your wellness goals and require only episodic visits. However, if you are seeing Cynthia or Rebekah for hormones or require prescription refills you may need follow up intermittently for lab work and to ensure you are on the correct dose. These office visits may be available via telehealth or phone call. There will be a charge for these visits.

  • What is your payment policy and fees for service?

    Payment is expected at the time of the appointment. We do accept Visa and Mastercard. We do not accept checks.
    Although individual fees exist for specific procedures or tests, most of the expense for medical care is based on time spent with you. Please be aware that one to three hours is spent reviewing your case before and after your visit and is included in the overall cost.

    All new patients have a comprehensive functional medicine evaluation. We will hold your first appointment with $150 deposit by credit card. This deposit is non-refundable in the event you cancel or reschedule less than 2 business days before your scheduled appointment or do not appear for your appointment.

  • Do I still need a primary care provider?

    Cynthia or Rebekah work as a functional medicine consultant and are not considered a replacement for your primary care provider. They work in conjunction with your health care team. We are not available for same-day emergency appointments. We do not have a 24-hour answering service and do not take calls on weekends. If you need to be seen urgently or have a medical emergency, you would need to visit an ER or urgent care center. We recommend all new patients with multiple medical conditions to keep their primary care provider to cover urgent visits.

  • Can I email my provider?

    If an email question or request is deemed by one of our staff to be too complicated or involved for a response via email, they may require that the patient schedule an office visit or telemedicine visit. Please also note that in some cases, it may take up to 48 hours to respond to messages. They should not be used for emergency or urgent questions.

  • Will my insurance cover my visits?

    We do not bill insurance companies.  We will provide you with a detailed billing summary that you can submit to your insurance for reimbursement.  We are considered out-of-network providers.   We will provide you with documentation for your (Health savings Account) HSA account.

  • Will insurance cover my lab testing?

    Most lab tests ordered will be billed directly to your insurance by the lab facility. Some lab tests we offer as part of our functional medicine testing program may not be covered by insurance. In this case you will be charged for the test by the lab and the office will charge a fee for processing the test. We will make every effort to make you fully aware of any additional lab fees that may be incurred and to give you the opportunity to choose which lab tests you feel comfortable paying for. You are under no obligation to purchase any specialty testing.

  • Can I get reimbursement from my insurance company?

    This will depend on your contract with your insurance company. When your insurance does reimburse, we are not able to guarantee how much you will receive. Medicare will not reimburse for care delivered by a non-participating medical provider.

  • How do I handle insurance company disputes?

    Your contract is between you and the insurance company. We are not able to get involved with disputes with your insurance company.  We will provide you with a detailed billing summary that includes appropriate diagnostic and therapeutic codes that you can submit to your insurance. We are not able to provide requests for medical records to your insurance company.

  • Cancellation policy

    Please cancel an appointment at least 48 hours in advance. We do not overbook or double book our providers and their time is reserved exclusively for you.  This also helps ensure that we can meet the needs of all our patients who are waiting to get an appointment. There will be a $100 cancellation fee for returning patients who fail to reschedule their appointments within 48 hours or no-shows.
